 T2 Clunk

PRi-T T2. The acute C6 was corrected in 3 visits and the 20 degrees of kyphosis reduced which left the neck perfectly straight(military) so i dropped down to T2 which wasn't tender at all(and pt is nearly asymptomatic); and the scope indicated pressure on the closed wedge suggesting a more chronic disc. When the scope shows a reduction another post Lat cervical will be taken to ess the lateral C-curve. if T2 doesnt correct the curve the next most likely candidate will be the chronic C4, then the atlas. i coined the term "cord pressure kyphosis" when i realized that the upper cervical spine commonly causes the kyphotic necks i had been trying to correct (erroneously) with a lower cervical/dorsal due to being mis-informed by my mentors. You can hear both facets pop before the clunk which is acceptable to call a clunk(or ca-nunk as Dr.G sometimes did).

 Pre/Post Xrays

i took both of these films on the 3rd visit 10 min apart(see"Cord Pressure ASLA" for the visits that preceeded this pre film). The 3rd visit i adjusted it as an ASLP because the palpation indicated the change in rotation(even tho both pre's showed ASLP), then took this post film. i had the lat flexion corrected(pre) from the original film not shown here. i have very similar xrays on another patient and dropped her hyper-estrogen from 400-41 with pre/post xrays and subsequent blood tests(days later) to prove it and she was hyper for years! surgeons have a surgery for these patients where they cut a piece of the mandible out so the teeth line up...scary and unecessary! Note the C2 spinous compensating PRS on the pre and in line with the dens and C3 spinous on the post...thats why you cant trust the xray especially in the acute spastic case. "The upper cervical spine is the master compensating area of the spine"-Dempstead My palpation skills in the upper cervical have come a long way and i'm still not anywhere near comfortable even now, just barely proficient. Cant argue with results like these tho.

 Atlas Adjustment Rotation

"Now when that's adjusted, it'll take up this little slack right here"-Gonstead Dr.Gonstead instructing on the proper ammount of rotation used when setting an atlas. Then he go's on to instruct that an atlas adjustment without this rotation wont move well. Adjusting an atlas straight across without this action is an MPi move and is quite traumatic to the patient in the form of shearing forces.

 "Leading" Instruction

This clarifies my stance to the critics of my C2 PRi-La adjustment vids (Quick Clunk and B-List Adjustments). Followers of all three seminars have brought this contention of "leading" to my attention but it's not always contraindicated and here is one of the exceptions. Perhaps those same critics are taking instruction from someone other than Gonstead and didn't watch this clip the 1000's of times necessary to understand exactly what the master is trying to get across. Dont underestimate Dempstead...When i roll in on a cervical i can crack that thunder in any vector with ample reserve. if my wrist looked like it was leading, i had absolute control of the degree in the moment of a millisecond. i also had justification from the "horses mouth" and the thousands of hours of practicing to control the precise vector and tangent. Perhaps the critics should talk less and save that mind energy that is otherwise wasted on thier misguided opinion...And instead, learn a thing or two about true Gonstead. You have to master the basics of the technique before you can even begin to add that something extra to the mix...

 PRS-Sp Double Thumb

Notice the distinct clunking sound. The SCP was the inferior right tip of the spinous process with the right thumb. The left thumb was contacting the right thumbnail and only the right thumb torqued although both thumbs delivered the thrust simultaneously.

 Stabilization Instruction

The difference between cracking and clunking a cervical(correctly) is in the stabilization. Watch the things Gonstead corrects in this stabilization and to what degree. DrGonstead moved this doctors thumb to the angle of the jaw but he moved his thumb back. Also the importance of stabilizing under the contact.
